What Is Penis Enlargement?

Penis enlargementmedically, penile augmentation – is a surgical procedure for permanently changing the length and girth of the penis. It typically consists of two components: penis lengthening and penis girth enhancement.

The goal is a permanent change in length and girth – with a natural, proportionate result and without impairing function.

At UGRS Center Darmstadt, penis enlargement is performed personally by Prof. Alessandro Littara or Dr. med. univ. Roberto Melone – two of the most experienced surgeons in this field in Europe.

Methods of Surgical Penis Enlargement

In practice, we distinguish between three approaches. In most cases we recommend combining lengthening and girth enhancement – not to change as much as possible, but to achieve a proportionate overall result.

Surgical Penis Lengthening

Part of the penis naturally lies inside the body – not visible from the outside, but present. By precisely adjusting the ligament structures in the pelvic area, combined with an internal flap technique using the body's own material, this internal portion can be partially relocated outward. We don't lengthen from the outside: we make existing length visible. The result shows both in the flaccid and the erect state[16].

Surgical Penis Girth Enhancement

For girth enhancement, we exclusively use the patient's own fat tissue – taken, for example, from the inner thigh or lower abdomen. The fat is processed and introduced beneath the penile skin. We do not use foreign materials such as silicone implants[1][6]. This approach follows the current guideline recommendations of the European Association of Urology (EAU). The final result develops over several months[19][20].

Combining Both Methods: Surgical Penis Enlargement

An isolated lengthening can look visually slimmer, while girth enhancement alone without an adjustment in length can appear unbalanced. Combining both allows the overall result to be controlled more precisely – which is why it is the better choice in most cases[1][6].

Achievable Results

Realistic benchmark values from our clinical practice, based on many thousands of procedures performed. Results following our treatment concept:

  • Lengthening: approx. 3.8 to 8.1 cm permanent
  • Girth increase: approx. 2 to 4 cm permanent

These figures are not a guarantee – the change actually achievable depends on individual anatomy, the starting situation, and tissue elasticity. The surgeon's experience significantly increases the reliability of the result[10][11].

What we know from practice: Many patients report a changed sense of their own body and greater confidence in intimate situations after the procedure.

What Does Penis Enlargement Cost – And What Does It Really Cost?

Costs depend on the procedure chosen. As a guide:

  • Combination of lengthening + girth enhancement (recommended): approx. €12,300
  • Lengthening only: approx. €9,500
  • Girth enhancement only: approx. €7,500
  • Enlargement in the flaccid state only: approx. €8,600
  • Corrective surgery (after a procedure elsewhere): approx. €13,500
  • Corrective surgery where materials not approved for penile use (e.g. silicone sheets, silicone balls) were used: approx. €14,500

The price includes medical consultation, the surgical procedure, anaesthesia, use of the operating facilities, and medical aftercare. Flexible payment options are available on request.

Medical Context: What Penis Size Is Normal?

For orientation, we refer to the European condom standard DIN EN ISO 4074. This standard specifies that a condom sold in Europe must have a minimum length of 16 cm.

Why Men Consider Penis Enlargement

The decision for such a procedure is rarely spontaneous. In our daily practice, we see that behind the wish for change there is often a longer story – a persistent feeling of not being "enough" by comparison[63], or insecurity in intimate situations. Some men begin to avoid closeness. Others have been preoccupied with the topic for years without ever talking about it.

These thoughts are not unusual[63]. That's exactly why every treatment with us begins with an open conversation, before a procedure is even considered.

Why It Is Anatomically Possible to Enlarge the Penis

Surgical penis enlargement is not based on artificial modification, but on existing anatomy. Part of the penis naturally lies inside the body. By precisely modifying the ligament structures, this internal portion can be partially relocated outward[16].

For girth, skin, subcutaneous tissue, and connective tissue play a decisive role. Introducing the patient's own fat tissue allows the diameter of the penis to be influenced – natural to both touch and appearance[1][9][19].

Limits of Penis Enlargement – And When We Advise Against It

No operation can change penis size without limit. Skin, tissue, and internal anatomy set natural boundaries[5][7]. Our goal is not the maximum theoretical change, but a balanced result that is functionally stable, looks natural, and fits the body's proportions.

There are situations where we deliberately advise against a procedure – when expectations exceed what is medically realistic[4]or when the health prerequisites simply aren't met.

This approach is part of how we understand treatment: we want to help – and that sometimes means clearly saying no.

Risks and Possible Complications

Like any surgical procedure, penis enlargement carries risks. Typical, temporary reactions after the procedure include swelling, a feeling of tightness, and minor bruising[1][6].

In rare cases, complications can occur, such as infections, wound-healing disorders, or uneven results[5][6][32]. What matters isn't just the operation itself, but the entire treatment concept: careful planning, precise execution, and structured aftercare all contribute significantly to reducing risk[10][11].

How the Treatment Proceeds

The path to treatment always begins with a conversation – not with a procedure. You can contact us by phone, email, or WhatsApp, whichever you prefer. After the initial contact, you can either arrange a personal consultation or – if you're already well informed and feel confident – book a treatment date directly. Every enlargement is planned individually for the patient; thorough medical planning is always part of the process. We don't offer a standard programme.

The operation itself usually takes about 70 minutes. It is performed as an outpatient procedure under twilight sleep and local anaesthesia – general anaesthesia is not required. The sutures used dissolve on their own, so a separate appointment for suture removal isn't needed[13][14][15]. Most patients are surprised how uncomplicated the procedure turned out to be in hindsight.

In the first two days after surgery, rest is the priority. Swelling and a slight feeling of tightness are normal and subside quickly. In the following weeks, healing progresses well – everyday activities gradually become possible again without needing to hold back. From the third to fourth week onward, a stretching device can be used if needed to support healing.

After five weeks, sexual intercourse and sport are possible again without restriction. The final result then develops over several months – the body needs this time to permanently stabilise the result[1][20]. Patience during this phase pays off.

This didn't go unnoticed. In 2014, the British Guardian called Germany the world champion of penis enlargement – with direct reference to the work of our physicians. Prof. Littara remains to this day the only physician named by name in the EAU guidelines for this field[73].
